Job Title: Licensed Practical Nurse (LPN)

Location: Maryland/Virginia
Reports To: Director of Nursing / Nurse Supervisor / Manager

Job Summary:
The Licensed Practical Nurse (LPN) provides essential nursing care to patients in their homes under the supervision of a Registered Nurse (RN) or physician. The LPN is responsible for delivering high-quality, compassionate care, administering medications, monitoring patient conditions, and supporting the overall well-being of patients. This role requires adherence to the scope of practice as defined by the licensing authority and compliance with agency policies and standards.

 

Key Responsibilities:

Patient Care & Support

  • Provide direct nursing care to patients, including wound care, medication administration, and monitoring vital signs.

  • Assist in developing and implementing individualized care plans under the direction of an RN or physician.

  • Monitor and document patients’ conditions, progress, and responses to treatments, reporting any significant changes to the supervising RN or physician.

  • Educate patients and families on disease management, medication administration, and home care techniques.

Team Collaboration

  • Work closely with RNs, physicians, and other healthcare professionals to ensure comprehensive patient care.

  • Supervise and support Home Health Aides and CNAs, ensuring adherence to care plans and agency standards.

  • Participate in team meetings and case reviews to discuss patient progress and care strategies.

Documentation & Compliance

  • Maintain accurate and timely clinical records, including progress notes, medication logs, and care plans.

  • Adhere to agency policies, HIPAA guidelines, and confidentiality protocols.

  • Ensure compliance with state and federal regulations governing nursing practice.

Administrative Duties

  • Assist in orienting new staff and contributing to in-service training programs.

  • Maintain a flexible schedule to accommodate patient needs and visit requirements.

  • Notify the office of daily itinerary changes and maintain availability for contact during working hours.

 

Physical Requirements:

  • Ability to speak, read, and write in English or Arabic.

  • Capable of lifting, transferring, or assisting patients weighing up to 160 pounds, with or without mechanical devices.

  • Ability to stand, walk, climb stairs, bend, twist, and squat for extended periods (up to 80% of the workday).

  • Comfortable working in environments with potential exposure to blood or infectious body fluids, utilizing provided protective equipment.

Mental Requirements:

  • Ability to concentrate on detailed tasks despite frequent interruptions.

  • Strong organizational skills to follow and complete daily routines and responsibilities.

  • Capable of comprehending and applying professional education materials.

  • Emotional resilience to cope with the mental and emotional stress of the role.

Qualifications:

  • Graduate of an accredited LPN program with a valid LPN license in the state of [Insert State].

  • Minimum of 1 year of nursing experience, preferably in home health care or a related field.
