Job Title: Licensed Practical Nurse (LPN)

Reports To: Director of Nursing / Nurse Supervisor / Manager
Job Summary:
The Licensed Practical Nurse (LPN) provides essential nursing care to patients in their homes under the supervision of a Registered Nurse (RN) or physician. The LPN is responsible for delivering high-quality, compassionate care, administering medications, monitoring patient conditions, and supporting the overall well-being of patients. This role requires adherence to the scope of practice as defined by the licensing authority and compliance with agency policies and standards.
Key Responsibilities:
Patient Care & Support

Provide direct nursing care to patients, including wound care, medication administration, and monitoring vital signs.

Assist in developing and implementing individualized care plans under the direction of an RN or physician.

Monitor and document patients’ conditions, progress, and responses to treatments, reporting any significant changes to the supervising RN or physician.

Educate patients and families on disease management, medication administration, and home care techniques.
Team Collaboration

Work closely with RNs, physicians, and other healthcare professionals to ensure comprehensive patient care.

Supervise and support Home Health Aides and CNAs, ensuring adherence to care plans and agency standards.

Participate in team meetings and case reviews to discuss patient progress and care strategies.
Documentation & Compliance

Maintain accurate and timely clinical records, including progress notes, medication logs, and care plans.

Adhere to agency policies, HIPAA guidelines, and confidentiality protocols.

Ensure compliance with state and federal regulations governing nursing practice.
Administrative Duties

Assist in orienting new staff and contributing to in-service training programs.

Maintain a flexible schedule to accommodate patient needs and visit requirements.

Notify the office of daily itinerary changes and maintain availability for contact during working hours.
Physical Requirements:

Ability to speak, read, and write in English or Arabic.

Capable of lifting, transferring, or assisting patients weighing up to 160 pounds, with or without mechanical devices.

Ability to stand, walk, climb stairs, bend, twist, and squat for extended periods (up to 80% of the workday).

Comfortable working in environments with potential exposure to blood or infectious body fluids, utilizing provided protective equipment.
Mental Requirements:

Qualifications:
